Driver's Education Contract

By accepting the terms and conditions you agree to the following information:

This made and entered into on ___________________ by and between
__________________________, hereinafter known as “student,” and
__________________________, hereinafter known as “Parent/Guardian” and Southern Indiana Driving School
LLC, hereinafter known as “Southern Indiana Driving School.”
That student does hereby agree to begin the driver education course offered by Southern Indiana Driving
School and, in exchange, Southern Indiana Driving School agrees to provide student with 30 hours of
classroom training and hours of behind-the-wheel training with a Southern Indiana Driving instructor.
Student agrees to provide a valid drivers permit issued by the Indiana Bureau of Motor Vehicles prior to beginning
the driver education course, and student must possess a valid permit during the online course and to have it on
them for every driving lesson. Student understands and acknowledges that Southern Indiana Driving School
may deny instruction for failure to provide records requested by any reasonable request of Southern
Indiana Driving School.
Student/Parent/Guardian agrees to pay the sum of 415 dollars to Southern Indiana Driving School in exchange
for the complete driver's education program that is to be provided by Southern Indiana Driving School.
This includes 30 hour online course and 6 hours of behind-the-wheel instruction. Full payment is due when
driving begins with an instructor. NO REFUNDS after a payment has been made. 
Southern Indiana Driving School will issue a Certificate of Completion upon satisfactory completion of the entire
course of instruction provided that student's account is paid in full.
Southern Indiana Driving School and student further agree to the following terms and conditions:
Southern Indiana Driving School will schedule student for, behind-the-wheel-training at Southern Indiana Driving
School's discretion.
If a student pick-up for any driving session is outside of Clark County Indiana, an additional fee of $10.00 will be
charged for each pick-up service.
Southern Indiana Driving School shall charge a fee of $65.00 for each additional hour of individual lessons.
Southern Indiana Driving School shall charge a fee of $35.00 to conduct the Skills Waiver Test.
The fees outlined herein are subject to change and shall expire 90 days from the date of this contract. All fees
must be paid by credit card or cash. Checks will not be accepted.
Southern Indiana Driving School may change the driving schedule at any time for mechanical failure, unsafe
driving conditions due to inclement weather and other similar conditions requiring a delay. Student will not
bring any claim based on, nor will Southern Indiana Driving School be held responsible for, delays caused by
mechanical failure, unsafe driving conditions due to inclement weather, or any other condition outside the school's
control. Southern Indiana Driving School will make all reasonable efforts to provide as much notice as possible
for any such delays and cancellations.

Student/Parent/Guardian understands that certain hazards and risks are inherent in the operation of a
motor vehicle. Student does hereby specifically assume all such risks as may be incurred in the operations of a
motor vehicle during the course of instruction. Southern Indiana Driving School does maintain insurance with
liability coverage in the amount of One Million Dollars ($1,000,000) on all of its instruction vehicles. To the
extent not covered by such insurance policy, student/parent/guardian does hereby release Southern Indiana
Driving School, its owners, agents and employees, from any liability whatsoever, and from any and all manner
of action and actions, cause and causes of action, suits claims or causes of actions resulting or arising from any
damage or injuries suffered by the student during the course or any extension therefore. The student hereby
further agrees to indemnify and hold the Southern Indiana Driving School harmless from any claim made against
it for any damage or injury suffered by any person, company, corporation or the entity, growing out of the
student's operation of the Southern Indiana Driving School vehicle or as a result of the student's course of
instruction to the extent that such claim is not covered by the Southern Indiana Driving School's insurance
coverage.
Once student registers to start behind-the-wheel, student has 180 days to complete the full behind-thewheel
training. Due to state regulations, Southern Indiana Driving School will not allow students to continue
any course instructions past this date. After 180 days student will no longer be enrolled at Southern Indiana
Driving School. Student will forfeit their driver's education program and payment.
It is understood and agreed that Southern Indiana Driving School makes no guarantees that student will
successfully pass the BMV examination upon completion of this course. Southern Indiana Driving School does
agree to give its best efforts in teaching the student to meet all requirements set forth by the Bureau of Motor
Vehicles.
This contract constitutes the entire agreement between the school and the student/parent and or guardian
and no verbal representations, assurance or promises not contained herein shall bind the school or the student.
